Minnesota Life Portability Form

Employees of Pasco County Schools who were previously insured for Basic Core Term Life, Accidental Death and Dismemberment (AD&D) and Supplemental Life insurance coverage may elect to continue coverage for themselves and their eligible dependents.  You must complete and mail the "Option to Port Term Life" form to Minnesota Life Insurance Company within sixty (60) days of your termination of benefits.  Evidence of Insurability (EOI) is not required in order to continue your supplemental life insurance coverage after employment.  Contact Minnesota Life at 1-866-293-6047, if you have any questions concerning continuation of your basic core or supplemental term life insurance.   Click here to download the "Option to Port Term Life" form.
