JOB GOAL

This employee is responsible for planning, developing, and coordinating transition services for special populations.

ESSENTIAL DUTIES & RESPONSIBILITIES

The following statements are intended to describe the general nature and level of work being performed by individuals assigned to this position. These statements are not intended to be construed as an exhaustive list of all duties and responsibilities required of those in this position.

Planning:

  1. Planning, coordinating and monitoring agency collaboration.
  2. Planning work evaluation activities.
  3. Participating in the development and implementation of district activities related to the Florida Strategic Plan for Transition.
  4. Participating in the development and implementation of district procedures for transition.
  5. Participating in the application for and implementation of appropriate state and federal grants related to transition.
  6. Monitoring federal and state funded projects related to transition.
  7. Monitoring, and evaluating Transition Individual Educational Plans.
  8. Planning procedures for the collection of data for required state, district, and federal reports
  9. Assisting with planning for Partners in Transition and interagency council activities
  10. Assisting with development and implementation of the FAPE 22 program.

Coordinating:

  1. Coordinating and monitoring agency collaboration activities and requirements.
  2. Coordinating and monitoring comprehensive work evaluation activities.
  3. Assisting high school ESE teachers in the development and implementation of quality transition IEPs.
  4. Developing and monitoring mentoring and job shadowing activities for ESE students.
  5. Coordinating development of a transition manual and distribution of transition information to parents, students, teachers, and other staff members.
  6. Developing and implementing transition workshops for teachers and parents.
  7. Developing and updating content for a transition web site for teachers, parents, students, and agencies.
  8. Collaborating with agencies and postsecondary education programs to provide transition activities and procedures for students, parents, and teachers.

Evaluating and Reporting:

  1. Evaluating transition programs for ESE students based on state, district, or federal criteria.
  2. Preparing and submitting reports regarding transition services for ESE students as required by local, state, federal, and educational agencies.
  3. Monitoring and submitting required reports for grant proposals and projects related to transition.
  4. Monitoring compliance with necessary student transition forms, records, and activities.
  5. Planning and implementing student follow-up activities to help determine transition program needs.

Public Relations:

  1. Assisting with providing information to community groups, parents, and agencies regarding ESE programs.
  2. Conferring with ESE teachers, agency representatives, and community groups as requested.

KNOWLEDGE, SKILLS & ABILITIES

EDUCATION, TRAINING & EXPERIENCE

  • Bachelor's degree
  • Classroom teaching experience

CERTIFICATES, LICENSES & REGISTRATIONS

  • Valid Florida Certification in any exceptional student education field

PREFERRED QUALIFICATIONS

  • Master's degree or above
  • High school ESE classroom teaching experience

TERMS OF EMPLOYMENT

Salary and benefits shall be paid consistent with the District’s approved compensation plan. Length of the work year and hours of employment shall be established by the District.

Performance of the job will be evaluated in accordance with provisions of the School Board’s policy on evaluation of personnel.
