School Health Records

Emergency Card

By law, an emergency card (electronic or paper) must be completed each school year by a parent/guardian. The school health staff reviews the information provided on the emergency card to assist the team in meeting the student’s healthcare needs. The information provided on the card is for confidential use only. The emergency card asks for the following information:

  • Demographic Information
  • Health Care Providers
  • Preferred Hospital
  • Contacts, including emergency contacts
  • Health conditions/medications/allergies
  • Consent for Health Care Services.

Student Cumulative Health Record

School health records are maintained throughout the student’s academic career and, by law, must be kept with the student’s accumulative school records. They are open to the student’s parent/guardian and to the student if eighteen or older.
