Premier Mobile Dental Van Program
The following services are offered on the Mobile Dental Van:
- Cleanings
- Fluoride treatments
- Sealants
Dental health services are offered to students in Kindergarten through 5th-grade students. If you would like your student to receive services, please complete the Premier Dental consent form. The consent form will be sent home with your student, or you may request a copy from the student's school clinic. Students must have a completed consent form on file to be seen on the Premier Dental Mobile Van. The scheduled dates, times, and school locations for the mobile dental van are listed below.
All students may be seen regardless of insurance status and in accordance with their insurance plans. For more information call Mobile Manager at 352-518-2000 ext. 9753.
Complete the Premier Dental consent as soon as possible in order to ensure that your student can be seen. Students must have a completed consent form to be seen on the Premier Dental Van. Consents will be sent home with your student or you can request a copy from the school clinic.
