Students with Diabetes 

The health, safety, and educational progress of a student with diabetes depends on cooperation and collaboration between the family and school staff members. Working together, they form the school health team that implements the provisions of the student's written plans and provides the necessary assistance in the school environment.

If a student’s physician authorizes a parent/guardian to adjust diabetes medication within specific limits, the registered school nurse will consider their input when applying these orders during school hours or school sponsored events. The student’s DMMP must also include a physician’s order allowing the nurse to make similar adjustments. While we value and consider your input, any final decisions about medication adjustments during school hours or events will be made by the registered school nurse, based on professional judgment and the physician’s orders. Please note that Clinic Assistants are not authorized to make or accept changes to medication plans.

  • A DMMP, Parent Authorization Form, and Self-Carry Form (if appropriate) will need to be completed upon enrollment and at the beginning of each school year. These forms can be found in the Forms tab to the right. 
  • Students who are identified as independent on the DMMP may provide their own routine diabetes management care.
  • If the student requires assistance managing their diabetes care, privacy should be available if the student or parent/guardian requests it.
  • The medical needs of the student with diabetes will be managed as a team effort between the medical provider, student, parent/guardian, school nurse, and trained clinic assistant/unlicensed assistive personnel. The student's diabetes management will be driven by the DMMP, Individualized Healthcare Plan (IHP), and Emergency Care Plan (ECP).
  • If you are requesting remote monitoring for your student reach out to your registered school nurse.
